By completing this form, you are subscribing to the Family Acute Subscription with Wave of Wellness, LLC. Acute conditions have a recent, rapid onset and differ from ongoing health conditions. Upon completion of this form, you will be required to complete a Consent Form for each family member as well as have a credit card on file through Square. Investment: $75 per month for a family of 4, each additional family member is $25 per month.

What is the Acute Family Subcription exactly?
This subscription is available to both non-clients and current clients year round. This includes unlimited acute* care with a 25 minute Zoom session and up to 5 days of care within the hours ranging from 10am-6pm eastern time.
How many family members are included?
This subscription pricing is for up to 4 family members at $75/month. Any additional family members (under the same roof) are $25 additional per person per month.
How many consultations are included?
You are essentially paying a subscription fee to have me on retainer should you anticipate needing care for your entire family. Instead of paying $50 per acute for non clients and $25 per acute for clients, this is a more economical option. Only true acutes will be able to receive care (any symptom that is re-current is NOT a true acute). If everyone in your family gets sick every week for a month straight, it’s covered. Please know that you will not receive a refund if you don’t require any acute care during the month.
